How is your life unfolding?

For each of the focus areas, circle the one answer that feels most true for you in this moment.

  1. Creativity
    1. I'm not an artist, so I must not be creative.
    2. I think I used crayons when I was a kid, does that count?
    3. I dabble in some creative hobbies like painting, scrapbooking or knitting in my limited free time.
    4. I feel pretty satisfied in my creative endeavors.
    5. I bring creativity into all that I do (e.g., problem solving on the job, personal expression and self-discovery, visioning, how I look at the world, etc.) It's a way of being.

    My favorite way(s) to express my creativity is through:

  2. Breath — Mind/Body/Heart/Soul Connection
    1. Most of the time I don't even notice my breath, my body or my emotions.
    2. I only notice my breath when I'm frustrated and let out a sigh or when I catch myself yawning.
    3. When I do pause to notice, I often find myself holding my breath, taking short and shallow breaths and/or having tension in my body.
    4. I have a fairly regular mediation, physical and/or spiritual practice to connect with my breath and calm my mind, body, heart and soul.
    5. I consciously use my breath to bring awareness to my mind, body, heart and soul. I create space daily for reflection so I can center, recharge and be fully present to my life.

    When I have a deep mind/body/heart/soul connection I am able to:

  3. Balance
    1. I don't feel like I have options around how I spend my time. I have so many roles and responsibilities and people are always counting on me.
    2. I sometimes overextend myself and find myself tired, burnt-out and resentful.
    3. I usually leave work at the office so I can at least have personal time after hours and on weekends.
    4. I occasionally schedule in "me" time despite my hectic calendar. I know that when I recharge my own batteries, I am more present in my life and for those around me.
    5. I make conscious choices every single day to say "yes" or "no" to things based on what's most important to me rather than what I think I "should" be doing.

    I feel most balanced when I:

  4. Goals
    1. Does my never-ending to-do list count as goals?
    2. Every January I come up with my new year's resolutions. I'll be on track for a few months and then life seems to always get in the way.
    3. I have a sense of what actions I need to take, but I haven't taken the time to write them down yet or move forward.
    4. I have written goals and I regularly check my progress.
    5. Not only do I have clearly defined goals, my goals align with my overall life vision. They include manageable action steps, specific timelines, resources needed and accountability. I'm making progress toward my goals and I celebrate my successes along the way.

    I'd be most happy and satisfied if I accomplished in the next six months.

  5. Meaning
    1. You mean there's more to life than just working for a pay check?!
    2. Sure, I may be successful, but I have a nagging feel like I'm just going through the motions of my life.
    3. I have a vague sense of what would bring more fulfillment and purpose in my life, but I'm not sure what to do about it.
    4. I've found ways to bring more fulfillment and meaning into my personal life (e.g., through volunteer work, relationships, hobbies, etc.), but not necessarily in my work life.
    5. I'm living my whole life in alignment with my purpose, values and vision. In my own way, I know I'm making a positive impact in the world.

    Assessment

    These responses are an opportunity for you to assess how you are currently choosing to live your life based on five major focus areas of my coaching approach: creativity, mind/body/heart/soul connection, balance, goals and meaning. The answers to each area are not necessarily in a strict linear progression as we may fluctuate at any given time between various states of being.

    After at least three months of coaching with me, though, clients tend to make statements more like the "e" answers than any of the other responses. These dedicated clients are now living a more whole life and are consciously making choices based on what's most important to them.
